Here is the additional information required for each student’s NYPD application.

• How many years have you lived at your current residence?
• From what year to now.

Please provide:

• Name of Employer
• Address
• Telephone
• Job Title
• Start and End date

Have you ever been fired, discharged, or terminated from employment?

If so, please provide:

• Name of the Employer
• Address
• Telephone Number
• Dates of Employment
• And the Reason for Discharge.

Please provide:

1. Copy of Valid State of Federal Photo Identification.
2. Birth Certificate / US Passport / US Military Record.
3. Social Security Card
4. Proof of Residence / Utility / Cable / Gas or Landline Phone. The most current bill (Must be within 6 Months )

Any cohabitant must include their email address and telephone number, as this information will be required to complete the online NYPD portal.

There is some documents given in the class that will also need to be notarized.

YOU WILL NEED A TOTAL OF 4 CHARACTER REFERENCES.
